I often use this generic pre-assessment form to gather information about clients. Note that you may create different pre-assessment forms based on information from your clients. For example, if a potential client contacts you for assistance with a back issue or an addiction, you could tailor the questions for those conditions.
Before my client walks through the door, I want to have the pre-assessment completed if at all possible. Sometimes, the client won’t return the form or be available for a call, and in those cases we have to do the best we can, but try to get the pre-assessment.
